It's probable balgruff may have killed his wife and mistress with the ebony blade to gain it power, then locked it up, and mehpala then wanted it's power released so she began spinning her own web to nelkir to get that power released.

But the dragonborn came along before that happened. Balgrufs wife is mostly dead by ebony blade which he locked away from everyone. TBH Balgrufs is my fav character in Skyrim.
